This release form is "required" for all pre-employment screenings.  (Forms available in client portal.)

1. Abuse Registry (Healthcare Workers):  This New Mexico registry provides information for any healthcare worker for investigations into allegations of abuse. 
3. Adjudication Services (Tribal):  Review of P.L. 101-630 along with FBI report and verifications for statute compliance for hiring recommendation.

7. Cannabis Industry Package – Employee:  Basic Package for hiring an employee in the Cannabis Industry. 
9. Cannabis Industry Package – Manager:  Mid-Level Package for hiring Managers.

13. County Civil Search:  Search of Plaintiff’s and defendant’s indexes for lawsuits pending and filed.
15. County Criminal Search:  Search for felony and misdemeanor records for the requested county.
17. Credit History:  This credit bureau report confirms name, address, and social security number as well as the applicant's credit history for employment or tenant purposes.
19. Document Retrieval: Obtaining actual documents for a case from the courthouse.

21. Education Verification:  Provides the most recent or highest degree of education and may include dates of attendance.
23. Employment Package – Single State:  Entry level package includes single state search only.
25. Employment Package – Multi State:  Entry level package includes multiple state searches.
27. Employment Verification:  Verifies previous term of employment, which may include dates of employment, position held, eligibility for rehire, etc.
29. Executive Package:  Extensive investigations for hiring Executives.
31. Fair Credit Reporting Act:  Providing a PDF of the Fair Credit Reporting Act.
33. Federal Bankruptcy Search:  This report is through the US Bankruptcy Court for bankruptcy searches.
35. Federal Civil Search:  Provides a search of plaintiff’s and defendant’s indexes for lawsuits pending and filed at a federal level.
37. Federal Criminal Search:  Provides a search for felony and misdemeanor records in Federal Courts.
39. Federal Package (Criminal, Civil & Bankruptcy):  Provides Criminal, Civil, and Bankruptcy information.
41. High Public Trust Package (Tribal): Provides upper level investigation for High Tribal Public Trust postition.  
43. Housing Package – Single State:  Entry level screening for tenants in a single states.
45. Housing Package – Multi State:  Entry level screening for tenants in multiple states.
47. Investigative Package (Customized):  A customized package based on specific company needs.
49. Miscellaneous Civil Search:  Search of Plaintiff’s and defendant’s indexes for lawsuits pending and filed.
51. Miscellaneous Criminal Search:  Provides a search for felony and misdemeanor records filed.
53. MVR (Motor Vehicle Record):  A verification of current driver history in the state requested, including infractions and verification of driver status and license classification.
55. Nationwide Arrest Record (NARS):  Search provides nationwide arrest records from a live database.
57. NM Package (Employment):  Entry – Level Package for a search of a single state investigation.
59. OIG-Office of Inspector General Search:  The OIG Exclusions Database search is for Federally funded programs that have been excluced for reasons including a conviction for Medicare or Medicaid fraud.
61. P.L. 101-630 (Tribal Package):  Public Law 101-630 is done for anyone who is working with or around children for the Tribal Community.
63. Personal References:  A telephone interview with an individual whose name and contact information has been provided by the client from the subject.
65. Professional Reference:  A telephone interview with a former supervisor from a previous employer whose name and contact information has been provided by the client from the subject.
67. Professional License Verification:  Provides verification of state or federal issued license.
69. Public Trust Package (Tribal) – NM: Provides entry level investigation for Tribal Public Trust postition in New Mexico only.
